Clinical Case Manager

Your Role & Responsibilities

As a Hospice Field Case Manager, you will play an essential role in delivering high-quality care to patients in both community and facility settings. Your responsibilities will include:

• Conducting comprehensive physical and psychosocial assessments to understand patient needs.
• Developing and implementing personalized nursing care plans to ensure comfort and quality of life.
• Communicating with physicians to request care plan modifications and written orders.
• Supervising, training, and evaluating home health aides and homemakers providing care.
• Electronically documenting patient visits for accurate and timely recordkeeping.

What We’re Looking For

• Associate’s Degree or Diploma from an accredited nursing program (BSN preferred).
• Minimum 2 years experience in medical-surgical, community health, and/or oncology nursing preferred.
• Active NYS RN license and valid NYS driver’s license required.
• Strong communication, teaching, and listening skills.
• Experience working in an interdisciplinary team environment.
• Proficiency in Word and Excel preferred.
• Ability to accurately input clinical data into electronic systems.
