Join our team as a Hospice Field Case Manager, where you’ll have the opportunity to provide meaningful end-of-life care in both community and facility settings.

About the Role

As a Hospice Field Case Manager, you will:

• Conduct physical and psychosocial assessments for MJHS Hospice and Palliative Care patients.
• Develop and implement nursing care plans tailored to patient needs.
• Provide support, education, and compassionate care to patients and their families.
• Electronically document visit occurrences to ensure accurate and efficient recordkeeping.
• Collaborate with an interdisciplinary healthcare team to optimize patient comfort and quality of life.
• Work one weekend per month and participate in backup On-Call coverage 2-3 times per month. (Car and escort service available for backup On-Call shifts.)

What You Need to Succeed

• Associate’s Degree or Diploma from an accredited nursing program; BSN preferred.
• 2+ years’ experience in med/surg, community health, and/or oncology nursing preferred.
• Active NYS RN license.
• Valid NYS driver’s license.
