Your Role & Responsibilities

As a Hospice Field Case Manager, you will provide specialized nursing care in both community and facility settings. Key responsibilities include:

• Performing comprehensive physical and psychosocial assessments for hospice and palliative care patients.
• Developing and implementing individualized care plans based on patient needs.
• Communicating with physicians to obtain necessary orders and adjust care plans accordingly.
• Supervising, training, and evaluating home health aides and homemakers to ensure quality care.
• Electronically documenting patient visits and maintaining accurate health records.

What You’ll Need

• Associate’s Degree or Diploma from an accredited nursing program (BSN preferred).
• At least 2 years’ experience in medical-surgical, community health, and/or oncology nursing preferred.
• Active NYS RN license and valid NYS driver’s license required.
• Strong communication, teaching, and interpersonal skills.
• Experience working as part of an interdisciplinary care team.
• Proficiency in Word and Excel preferred.
• Ability to efficiently input and manage clinical data.
